<p id="c6c2">The first time I suspected light could make people gain weight, I was not in a clinic.</p>



<p id="487a">I was standing in my kitchen at 1:07 a.m., the only illumination a cold rectangle from the refrigerator.</p>



<p id="b10e">It felt like a reversed Caravaggio scene. Darkness everywhere, a harsh pool of light on a plate of leftovers.</p>



<p id="5055">I was not hungry.</p>



<p id="857e">The light wired me awake. It felt like an invitation to eat.</p>



<p id="e6e3">I am a radiation oncologist.</p>



<p id="92f6">I discuss circadian clocks with patients more often than most in my specialty because I’ve seen, over decades, how sleep, light, food timing, stress, and movement influence recovery, inflammation, and weight.</p>



<p id="4469">When you sit in a treatment room for a long enough time, you start to notice patterns.</p>



<p id="fa7d"><mark>People who live in bright evenings and dim mornings often struggle with appetite, cravings, and maintaining a healthy body composition.</mark></p>



<p id="4e7d">They are swimming upstream against their biology.</p>



<p id="e4e2">This is the essay I wish someone had handed me ten years ago.</p>



<p id="58fa">Light is not neutral.</p>



<p id="bc4b"><mark>It is a metabolic signal.</mark></p>



<p id="33dd">And the way we dose it each day can quietly nudge our insulin, our melatonin, our leptin and ghrelin, our brown fat, and even the clocks inside our liver and pancreas.</p>



<p id="77e6">Let me show you how to use that to your advantage.</p>



<h1 class="wp-block-heading" id="0dd0">The Night I Realized Light Exposure Could Make You Fat.</h1>

<p id="e83b">In the clinic, I began asking a new question: not just how many hours you sleep, but how much light hits your eyes after sunset.</p>



<p id="6900">Most patients stared. Then came the stories: three glowing screens, bright LEDs in the bedroom, a dog walk under sodium street lamps.</p>



<p id="4210">Late light. Fragmented sleep.</p>



<p id="f17f">Late eating. Creeping weight.</p>



<p id="3d9f">Our fat cells tell time. So do our mitochondria. So does your gut microbiome.</p>



<p id="d0c1">Light at the wrong time scrambles those clocks. Scrambled clocks change how you store energy.</p>



<h1 class="wp-block-heading" id="2343">Your Fat Has a Clock: Circadian Rhythm, Metabolism, and Weight Gain.</h1>



<p id="395a">Every cell in your body keeps time.</p>



<p id="a855">Morning light anchors the master clock in your brain, which in turn syncs the clocks in your organs and fat cells.</p>



<p id="577a">Weak morning light and strong evening light throw those clocks out of phase.</p>



<p id="2911">The result is a subtle metabolic jet lag that never ends.</p>



<p id="08d3">Quiet jet lag doesn’t show up on your calendar. It shows up on your scale.</p>

<p id="1a7f">It cross-talks with insulin.</p>



<p id="181e">When melatonin levels are high, your ability to handle glucose decreases.</p>



<p id="e10f">That is adaptive if you are asleep.</p>



<p id="ac53">It is not adaptive if you are scrolling with a bowl of cereal near midnight.</p>



<p id="5e3b">Bright light at night suppresses melatonin, delays sleep, and shifts appetite later.</p>



<p id="3b99">You wake underslept, with more ghrelin, less leptin, and a stronger drive to eat ultraprocessed food.</p>

<p id="920c">Ten minutes of unfiltered outdoor light soon after waking can move your circadian clock earlier, deepen your sleep that night, and improve next-day insulin sensitivity.</p>



<p id="d072">Morning light is rich in wavelengths your brain needs to set the day. Indoors, even bright-looking rooms are often one or two orders of magnitude dimmer than outside.</p>



<p id="4e44"><mark>Your brain can tell the difference. So can your pancreas.</mark></p>



<p id="ed0d">If you do one thing after reading this, step outside within 30 minutes of waking.</p>



<p id="f00a">If it is cloudy, go anyway. If it is winter, aim longer. Treat it as you would a prescription.</p>

<p id="2218">Swap bedside LEDs for warm, low-lux bulbs.</p>



<p id="235c">Set your phone to grayscale and enable a screen sunset. Cover the power lights with black tape.</p>



<p id="fc99">Close the fridge quickly. Dim your home two hours before bed until it resembles a Rembrandt painting.</p>



<p id="7873">Your metabolism prefers Rembrandt over Times Square.</p>



<h1 class="wp-block-heading" id="993b">What I tell patients in the clinic</h1>

<li>See the morning light early and consistently.</li>



<li>Eat most of your calories in the daylight zone.</li>



<li>Dim the house two hours before you want to sleep.</li>



<li>Stop eating at least two to three hours before going to bed.</li>



<li>Keep the bedroom dark, cool, and free of screens.</li>



<li>Anchor wake time seven days a week.</li>



<li>Move your body in the day, not at midnight.</li>



<li>Treat shift work like altitude. You need extra recovery, extra discipline, and a plan.</li>
</ol>

<h1 class="wp-block-heading" id="b445">How this plays out in the real world</h1>



<p id="2a60">A patient with breast cancer told me she could not lose weight despite “doing everything right.”</p>



<p id="ac05">She tracked calories, lifted weights, and avoided ultraprocessed foods. She also answered emails at midnight under bright LED downlights and ate a second dinner at 10:30 p.m.</p>



<p id="3d6c">We moved her dinner to 6 p.m., instituted a house-wide dim at 8 p.m., added morning light exposure, and asked her to maintain a stable wake time, even on weekends.</p>



<p id="8c6b">Six weeks later, she had lost six pounds without changing her total calorie intake.</p>



<p id="4150">Her sleep improved. Her cravings diminished.</p>



<p id="755f">The scale finally listened.</p>

<p id="07e4">We are the first species to flood the night with light and the day with dimness.</p>



<p id="278b">We built a 24-hour culture and then wondered why our biology pushed back.</p>

<p id="db38">I ate more than usual.</p>



<p id="61a2">I felt full every day.</p>

<p id="d897">Two pounds lighter.</p>



<p id="e3f9">My steps had not changed.</p>



<p id="06d5">My sleep was the same.</p>



<p id="2707">I tracked nothing.</p>



<p id="b034">I restricted nothing.</p>



<p id="6ae1">Yet somehow, my body felt clearer, lighter, and calmer.</p>



<p id="16cf">That was the moment I understood.</p>



<p id="f2ac">The issue was not how much I had been eating.</p>



<p id="1183">It was what I had been eating.</p>

<h1 class="wp-block-heading" id="b25c">The Research Now Makes It Obvious</h1>



<p id="b10f">A major review published in&nbsp;<em>Nature Reviews Endocrinology</em>&nbsp;confirmed what I had already suspected.</p>



<p id="dc4b">Ultra-processed foods, often called UPFs, are not just harmless conveniences.</p>



<p id="df27">They actively promote overeating, disrupt metabolism, and increase the risk of obesity and chronic disease.</p>



<p id="27e3">Researchers reviewed decades of data.</p>



<p id="6983">Their conclusions were clear:</p>



<ul>
<li>UPFs override your sense of fullness</li>



<li>They damage the gut microbiome</li>



<li>They interfere with hormones like insulin and leptin</li>



<li>They promote chronic inflammation</li>



<li>They increase the risk of early death</li>
</ul>



<p id="a52c">These are not neutral calories.</p>



<p id="f5d8">They are chemical provocations.</p>

<p id="e323">A century ago, food processing helped prevent disease.</p>



<p id="ceee">Preservatives and fortification solved real problems like spoilage and vitamin deficiencies.</p>



<p id="90ff">But today’s ultra-processed foods are something else entirely.</p>



<p id="14f8">They are stripped of fiber and water.</p>



<p id="e0d0">They are saturated with sugar and salt.</p>



<p id="4b0b">They are softened or crisped for speed.</p>



<p id="0a3f">They require little chewing and deliver maximum pleasure in minimal time.</p>



<p id="24e8">These are not foods made to nourish.</p>



<p id="ccde">They are products made to disappear.</p>

<p id="e9dc">In Penang and Singapore, I kept waiting for the usual signs.</p>



<p id="11de">Cravings.</p>



<p id="5177">Energy crashes.</p>



<p id="24a9">That pull toward sugar at night.</p>



<p id="1cb2">None of them came.</p>

<p id="10ef">Greens stir-fried in oil.</p>



<p id="b64f">Fish with bones intact.</p>



<p id="7d4e">Rice, fruit, spice, and broth.</p>



<p id="8d0f">Even the desserts had substance.</p>



<p id="c338">Even the snacks had structure.</p>



<p id="076e">And every bite required chewing.</p>



<h1 class="wp-block-heading" id="4148">What Ultra-Processed Foods Really Do</h1>



<p id="6a36">The review and related studies show that UPFs:</p>



<ul>
<li>Speed up how fast we eat</li>



<li>Bypass satiety signals</li>



<li>Starve the beneficial bacteria in the gut</li>



<li>Trigger a hormonal imbalance</li>



<li>Promote low-grade inflammation</li>



<li>Crowd out nutrient-rich foods</li>
</ul>



<p id="d66c">One U.S. study linked high UPF intake to more than 120,000 preventable deaths each year.</p>



<p id="8055">Another found strong associations with cardiovascular disease, type 2 diabetes, and cognitive decline.</p>



<p id="5fb6">This finding is not just about gaining weight.</p>



<p id="b4e6">It is about how these foods affect nearly every system in the body.</p>

<h1 class="wp-block-heading" id="d6df">Labels Lie. Chemistry Tells the Truth.</h1>



<p id="d05b">UPFs often pretend to be healthy.</p>



<p id="d45b">They say&nbsp;<em>low-fat</em>,&nbsp;<em>gluten-free</em>,&nbsp;<em>plant-based</em>, or&nbsp;<em>high-protein</em>.</p>



<p id="16be">But flip the package and the label tells a different story.</p>



<p id="aad3">You’ll find artificial flavors, gums, starches, stabilizers, and sweeteners you can’t pronounce.</p>



<p id="0969">You’ll find them in:</p>



<ul>
<li>Protein bars</li>



<li>Ready-to-drink shakes</li>



<li>Flavored yogurts</li>



<li>Cereal with cartoon characters</li>



<li>Shelf-stable snacks that never spoil</li>
</ul>



<p id="d4da">These foods don’t just deliver calories.</p>



<p id="8de0">They confuse your hunger.</p>



<p id="8f9e">They dull your instincts.</p>



<p id="4046">They keep you coming back.</p>



<h1 class="wp-block-heading" id="9b45">What I Now Tell My Patients</h1>



<p id="ce18">Instead of strict food rules, I offer simple replacements.</p>



<ul>
<li>Eat foods with fewer than five ingredients</li>



<li>Choose oats over cereal</li>



<li>Choose fruit over a bar</li>



<li>Choose nuts over crackers</li>



<li>Cook when you can, even if it’s just olive oil, garlic, and greens</li>
</ul>



<p id="7c67">Real food asks you to chew.</p>



<p id="db3f">It slows you down.</p>



<p id="7871">It satisfies you in a way engineered foods cannot.</p>



<p id="ad89">You don’t need to be perfect.</p>



<p id="411b">You only need to shift direction.</p>



<h1 class="wp-block-heading" id="986c">Letting Go Is Hard for a Reason</h1>



<p id="f55e">Manufacturers design ultra-processed foods to be addictive.</p>



<p id="0ffe">They melt in your mouth.</p>



<p id="6f41">They hit the same reward centers as nicotine.</p>



<p id="da80">They are easy to chew and hard to resist.</p>



<p id="3eb7">They make you eat faster and feel less full.</p>

<li>Add a vegetable to each dinner</li>



<li>Keep one snack unprocessed</li>
</ul>



<p id="40a3">The smallest shift creates momentum.</p>



<p id="86ba">And momentum is what makes change last.</p>



<h1 class="wp-block-heading" id="4bd4">Final Thoughts: Hunger Is a Signal</h1>



<p id="384d">In Malaysia, I stopped thinking about food.</p>



<p id="0fc9">I didn’t count macros.</p>



<p id="c0f8">I didn’t obsess over protein.</p>



<p id="04ee">I ate when I was hungry and stopped when I was full.</p>



<p id="e54f">Because the food I was eating allowed me to stop.</p>



<p id="518c">When I walked through the airport food court back home, everything looked different.</p>



<p id="bcc3">It wasn’t the smell that hit me.</p>



<p id="09c5">It was the stillness.</p>



<p id="d952">The silence.</p>

<p id="51af">I came to Borneo to let the ground fight back.</p>



<p id="83d3">This morning, outside Kota Kinabalu, Malaysia, my boots slipped on a tangle of wet roots.</p>



<p id="20dd">The air smelled of earth and rain.</p>



<p id="76db">A quiet hum of life surrounded me.</p>



<p id="f46c">In that awkward, humbling stumble, I felt something rare:</p>



<p id="0abe"><strong>My body woke up.</strong></p>



<h1 class="wp-block-heading" id="c545">The Comfort Trap That’s Making Us Weak</h1>



<p id="935f">In Seattle, I walk sidewalks and treadmills — smooth, safe, and sterile.</p>



<p id="112b">For most of human history, humans moved across shifting ground.</p>



<p id="9665">Forests. Fields. Hills.</p>



<p id="c488">Every step required micro-adjustments.</p>



<p id="1de2">Your feet, ankles, and hips worked together without your conscious effort.</p>



<p id="a715">But today?</p>



<p id="e507">Flat concrete. Cushioned shoes. Escalators and elevators.</p>



<p id="5164">The result?</p>

<p id="ea4d">Or just the wind reminding me to stay alert?</p>



<p id="d181">Either way, I wasn’t scrolling my phone.</p>



<p id="2c0a">You can’t walk distracted when the jungle might bite back.</p>



<p id="b21b"><strong>Uneven ground forces presence.</strong></p>



<p id="32c4">And in a world addicted to autopilot, presence is a rare medicine.</p>



<h1 class="wp-block-heading" id="8451">Rough Terrain Rewires Your Brain for Survival</h1>



<p id="24be">I used to think walking was the ultimate autopilot activity.</p>

<p id="ee62">However, the danger begins decades earlier, when our feet stop adapting and our brains stop noticing.</p>



<h1 class="wp-block-heading" id="d793">How I Brought This Home</h1>



<p id="478f">Back in Seattle, I made some simple shifts:</p>



<p id="796f">✔️ I walked forest trails instead of smooth streets.<br>✔️ Hiked on pebble beaches where every step shifted.<br>✔️ Walked barefoot on grass during morning routines.<br>✔️ Kept a wobble board under my standing desk.<br>✔️ Balanced on sidewalk curbs instead of staying in the middle of the path.</p>

<p id="0485">Choose trails appropriate for your skill level, watch your footing, and skip barefoot walking where sharp rocks or hidden hazards could cause injury.</p>



<h1 class="wp-block-heading" id="0eae">How to Start (No Jungle Required)</h1>



<p id="6c92">Here’s how I coach patients and friends to rebuild strength and balance:</p>



<p id="8fb1">✔️&nbsp;<strong>Choose uneven surfaces:</strong>&nbsp;dirt trails, grassy parks, rocky beaches, or mulch paths.<br>✔️&nbsp;<strong>Go barefoot (when safe):</strong>&nbsp;start on grass, packed earth, or soft sand.<br>✔️&nbsp;<strong>Slow your pace:</strong>&nbsp;give your body time to adapt.<br>✔️&nbsp;<strong>Add balance drills:</strong>&nbsp;balance on logs, low curbs, or balance cushions indoors.</p>



<p id="0eed">You don’t need to overhaul your life.</p>



<p id="f011">But you do need to stop walking only on flat, predictable ground.</p>



<h1 class="wp-block-heading" id="5a97">Adaptation is the Goal — Not Perfection</h1>



<p id="ccc3">The real danger isn’t falling.</p>



<p id="22ce">It’s losing your ability to adapt.</p>



<p id="3dd1">On uneven ground, your body learns to adjust.</p>



<p id="51f1">Your brain stays alert.</p>



<p id="1a6c">Your nervous system sharpens.</p>



<p id="a9b1">It’s not about avoiding every stumble.</p>



<p id="1749">It’s about&nbsp;<strong>responding better when you do.</strong></p>



<h1 class="wp-block-heading" id="7262">What Happens If You Ignore This?</h1>



<p id="a3bf">If you wait until your balance is depleted, regaining it becomes harder.</p>



<p id="6271">The loss happens slowly:</p>

<p id="e8e1">By the end of my hike, my legs were burning, and my boots were caked with mud.</p>



<p id="9e74">But my mind felt clearer than it had in months.</p>



<p id="f4b8">Maybe this is what we’re wired for:</p>



<p id="2615">Not flat streets. Not endless comfort.</p>



<p id="d126">But uneven steps that challenge and strengthen us.</p>



<p id="a60d">Modern life has made our paths smooth.</p>



<p id="ae51"><mark>But with a little intention, we can rebuild what we lost — balance, strength, awareness.</mark></p>

<p id="0e76">But for us, it was something simpler.</p>



<p id="9011">A stroke.</p>



<p id="26f7">And life was never the same.</p>



<p id="0bc8">My uncle survived his stroke, but lost half his body to paralysis.</p>



<p id="57b7">But now, with all my training, stroke feels like an enemy we still haven’t defeated.</p>



<p id="2500">Until maybe now.</p>



<h1 class="wp-block-heading" id="c09c">Every Minute Counts — And Millions of Brain Cells Die Waiting</h1>



<p id="2280">Doctors have a saying:</p>



<p id="d449"><strong>“Time is brain.”</strong></p>



<p id="a7ed">For every minute your brain goes without blood flow, you lose almost&nbsp;<strong>2 million neurons</strong>&nbsp;— and about a week of independent life.</p>



<p id="fe91">Most strokes are ischemic — caused by a clot that blocks blood flow to the brain.</p>



<p id="2831">The clot may form in place (<strong>thrombotic stroke</strong>) or travel from elsewhere (<strong>embolic stroke</strong>).</p>



<p id="0b89">Today, we treat them with two main tools:</p>



<ul>
<li>With clot-busting drugs like tPA.</li>



<li>Or by physically removing the clot with a device snaked into the brain.</li>
</ul>



<p id="be4e">But here’s the harsh truth:</p>



<p id="8d19"><strong>Even today’s best devices fail on the first try about half the time.</strong></p>



<p id="fbdb">And the longer it takes, the worse the outcome.</p>



<h1 class="wp-block-heading" id="42df">A Spinning Breakthrough: The Milli-Spinner</h1>



<p id="ba63">Now, something new.</p>



<p id="22fa">At Stanford, engineers built a tiny device called the&nbsp;<strong>milli-spinner.</strong></p>

<p id="ae75">Over time, I’ve come to believe that&nbsp;<strong>chronic inflammation is the common thread behind most chronic diseases</strong>. The evidence keeps stacking up.</p>

<p id="fa7c">Dementia? Check.</p>



<p id="f832">Type 2 diabetes? Check.</p>



<p id="b9ab">Heart attacks. Strokes. Obesity. Cancer?</p>



<p id="56d3">Check, check, check.</p>



<p id="22b9">We call these conditions separate diagnoses.</p>



<p id="25ae">But I’ve come to see them as symptoms of a deeper cause: the body’s immune system stuck in the “on” position.</p>



<p id="d8b8">Scientists have even coined a term:&nbsp;<em>inflammaging</em>&nbsp;— the chronic, low-grade inflammation that accelerates aging and shortens lifespan.</p>



<p id="2d5d">That realization changed the way I live.</p>

<h1 class="wp-block-heading" id="0735">6 Ways I’m Extinguishing the Fire</h1>



<p id="3307">I don’t believe in silver bullets. But I do believe in stacking small daily wins. Here’s how I’m pushing back against chronic inflammation:</p>



<h2 class="wp-block-heading" id="fac5">1. Sleep Like It’s Medicine</h2>



<p id="7055">Sleep isn’t optional. It’s therapeutic.<br>Insufficient sleep increases inflammatory markers, such as C-reactive protein. But quality sleep can lower them, especially when it becomes a consistent, protected ritual.</p>



<p id="86b3">I now aim for 7.5–8 hours per night, without apology. I treat it like an appointment; I don’t cancel.</p>



<h2 class="wp-block-heading" id="3475">2. Move Daily, Not Occasionally</h2>



<p id="4670">Exercise cools inflammation, especially the aerobic kind.<br>Walking. Cycling. Swimming. Even light strength training. They all lower pro-inflammatory cytokines and boost your cellular resilience.<br>And yes, I consider walking to be medicine.</p>



<h2 class="wp-block-heading" id="b209">3. Eat Food That Fights for You</h2>



<p id="2b32">I’ve shifted toward a Mediterranean-style diet, which includes olive oil, vegetables, nuts, berries, and fatty fish.<br>I’ve also added turmeric, green tea, and fiber-rich legumes.<br>These foods don’t just fuel me. They&nbsp;<em>protect</em>&nbsp;me from the inside out.</p>



<h2 class="wp-block-heading" id="b74b">4. Avoid the Fire-Starters</h2>



<p id="4b65">Two habits pour gasoline on inflammation:</p>



<ul>
<li><strong>Smoking</strong></li>



<li><strong>Excess alcohol</strong></li>
</ul>



<p id="5b5d">I’ve never smoked. But I do drink on occasion — and now I limit that to one glass a week, if that. Most of the time, sparkling water does the trick just fine.</p>



<h2 class="wp-block-heading" id="cfbc">5. Stay Lean for the Right Reason</h2>



<p id="08c5">Weight isn’t just cosmetic — it’s biochemical.</p>



<p id="bbe8">Visceral fat (the kind that wraps around your organs) fuels inflammation and increases risk for cancer, dementia, and heart disease.</p>



<p id="853e">I stay lean, not for a mirror, but for my mitochondria.</p>



<h2 class="wp-block-heading" id="b56c">6. Take Oral Health Seriously</h2>



<p id="3123">Inflamed gums = inflamed body.</p>



<p id="18e7">I didn’t always take flossing seriously. But the link between periodontal disease and heart disease — even cognitive decline — is real.</p>



<p id="3004">Now I treat my toothbrush like a prescription.</p>

<p id="1391">Aging is inevitable.</p>



<p id="6a76">But&nbsp;<em>how</em>&nbsp;do we age?</p>



<p id="db48">That’s far more flexible than most people realize.</p>



<p id="df57">If you want more energy, sharper cognition, and a lower risk of disease, start by cooling the flame of inflammation.</p>



<p id="c03d">It’s not glamorous. But it works.</p>

<p id="1f5b">And how we treat the quiet signals our body sends us — before they become sirens.</p>



<p id="6c3b">You don’t need a prescription to start.</p>

<p id="5224">We’re told that foods like blueberries are miracle cures — that if we just eat clean enough, we can outrun stress.</p>



<p id="7a0d">But here’s what I’ve seen in practice:</p>

<p id="d767">ORAC, short for&nbsp;<a href="https://goveganway.com/understanding-orac-values-antioxidants-levels/" rel="noreferrer noopener" target="_blank"><strong>Oxygen Radical Absorbance Capacity</strong></a><strong>&nbsp;</strong>measures how well a food can neutralize free radicals (unstable molecules that damage cells, accelerate aging, and promote inflammation).</p>



<p id="ff0c"><strong>Some of the&nbsp;</strong><a href="https://www.ars.usda.gov/news-events/news/research-news/1999/high-orac-foods-may-slow-aging/" rel="noreferrer noopener" target="_blank"><strong>highest-ORAC foods</strong></a><strong>&nbsp;include:</strong></p>

<p id="18f2">Consuming these foods regularly can make your body more efficient at extinguishing the “metabolic fires” triggered by stress, poor sleep, and inflammation.</p>

<p id="dd4b"><strong>But they’re not enough.</strong></p>



<h1 class="wp-block-heading" id="2770">Stress, Sleep, and the Limits of Diet</h1>



<p id="23ec">Take a real-world example:</p>



<p id="aa38">A 49-year-old entrepreneur came to me burned out.</p>



<p id="1e64">She exercised.</p>



<p id="b096">Ate mostly plants. Drank matcha. Took magnesium.</p>



<p id="c66a">Still exhausted. Irritable. Foggy.</p>



<p id="664e">Why?</p>



<p id="061b">She was sleeping five hours a night, answering emails at midnight, skipping meals, and never pausing.</p>



<p id="b111">Her nervous system was locked in a state of fight-or-flight.</p>

<p id="31f2">Here’s what I tell patients when they’re doing all the “right” things — but still feel off:</p>



<h1 class="wp-block-heading" id="cb5c">1. Anchor meals to rhythm, not mood</h1>



<p id="b1ef">Eat at consistent times daily. This stabilizes your gut clock and supports digestion.</p>



<h1 class="wp-block-heading" id="791a">2. Start the day with color</h1>



<p id="21ab">Aim for 3+ natural colors before noon: blueberries, spinach, turmeric, red pepper.</p>



<h1 class="wp-block-heading" id="ad26">3. Pair food with ritual</h1>



<p id="df12">Eat away from screens. Use real dishes. Go outside if you can. This activates your parasympathetic system.</p>



<h1 class="wp-block-heading" id="710e">4. Don’t supplement stress away</h1>



<p id="925a">Magnesium, ashwagandha, resveratrol — all useful. But only after the basics are covered: sleep, movement, light, and breath.</p>

<p id="8a58">When I asked him what sparked joy — my now-standard question for new consults — he didn’t hesitate.</p>



<p id="1bad">“Travel,” he said, his eyes lighting up. “Dozens of countries. I love learning how people live, eat, think.”</p>



<p id="3058">Then he paused. “But if I had to choose just one?”</p>



<p id="f941">He leaned forward, almost conspiratorially.</p>



<p id="5ddb">“Japan. Lived there over 25 years.”</p>



<p id="5241">I perked up. “Hontō ni?”</p>



<p id="5d42">“Eh? Hontō hontō!” he beamed.</p>



<p id="6690">And just like that, the oncology suite turned into an izakaya.</p>

<p id="5832">We chatted in Japanese for several minutes — I, a Black man from the Pacific Northwest with a Japanese wife and a daughter who had attended college in Kyoto; he, a white man with a surprising Tokyo accent and stories that could have filled a dozen ryokans.</p>



<p id="e546">The nurses outside the curtain must’ve been baffled.</p>



<p id="ba8c">There we were: two middle-aged men, dressed like surgical extras, speaking rapid-fire Japanese about onsen, natto, and konbini snacks.</p>



<p id="cf4f">In radiation oncology, these are the moments you don’t forget.</p>



<h1 class="wp-block-heading" id="9287"><strong>The Kind of Joy You Can’t Buy</strong></h1>



<p id="7b3f">When our conversation drifted back to English, we kept circling the same theme:&nbsp;<em>experience</em>.</p>



<p id="d0ac">He told me about sleeping in the Sinai desert under a blanket of stars.</p>



<p id="4fbf">About sipping strong coffee in Addis Ababa.</p>



<p id="79e7">About riding motorcycles through Southeast Asia before Google Maps existed.</p>



<p id="c445">What he didn’t talk about were things.</p>



<p id="3776">No fancy watches.</p>



<p id="36bb">No new Teslas.</p>



<p id="5e97">No gadgets.</p>



<p id="63f1">Just the texture of moments lived.</p>



<p id="d8ee">And it hit me:&nbsp;<mark>the joy that lit up his face wasn’t the kind you get from opening a box.</mark></p>



<p id="79cf">It was the kind you&nbsp;<em>earn</em>&nbsp;by stepping into the unfamiliar. The kind that asks something of you — and gives back more than it takes.</p>



<h1 class="wp-block-heading" id="9b77"><strong>The Science of Why It Feels So Good</strong></h1>



<p id="5cce">We tend to think happiness is about comfort.</p>



<p id="16de">But psychologists like Dr. Laurie Santos (of Yale’s wildly popular&nbsp;<a href="https://www.coursera.org/learn/the-science-of-well-being" rel="noreferrer noopener" target="_blank"><em>Science of Well-Being</em></a>&nbsp;class) suggest that&nbsp;<em>the happiest people spend less on stuff and more on experiences</em>.</p>



<p id="dd5d">Why?</p>



<p id="2a3d">Because of experiences:</p>



<ul>
<li>Give us stories we can retell</li>



<li>Bring us into contact with others</li>



<li>They are often tied to personal growth</li>



<li>Don’t lose their sparkle the way objects do</li>
</ul>



<p id="bf4d">A new phone gets old fast. But your first tuk-tuk ride in Bangkok? That stays with you.</p>



<p id="e649">There’s even a term for the trap we fall into with material things:&nbsp;<strong>hedonic adaptation</strong>.</p>



<p id="2dec">The idea is that we quickly get used to new pleasures.</p>



<p id="1a37">The house, the car, the clothes — they stop thrilling us.</p>



<p id="59ba">But experiences?</p>



<p id="24a0">They stay vivid.</p>

<h1 class="wp-block-heading" id="5246"><strong>My Favorite Journeys</strong></h1>



<p id="8808">Some of my favorite travel memories come from places that required a little more effort than, say, Paris or London.</p>



<ul>
<li>Egypt: navigating the frenetic, poetic chaos of Cairo traffic, then standing in stillness before the pyramids.</li>



<li>Turkey: sipping tea in the shadow of the Blue Mosque, hearing the call to prayer echo across centuries.</li>



<li>Japan: of course — always Japan — with its contradictions, its grace, its reverence for detail.</li>



<li>Indonesia: maybe my favorite of all, where time moves differently and kindness is a national trait.</li>
</ul>



<p id="e170">These places didn’t just offer a change of scenery. They offered a shift in&nbsp;<em>me</em>&nbsp;— the way I saw others, the way I understood culture, the way I experienced time.</p>

<h1 class="wp-block-heading" id="88fc"><strong>What Travel (and Cancer) Teaches Us</strong></h1>



<p id="1f44">My patient and I shared one more truth that day: that illness, like travel, strips you down to what matters.</p>



<p id="ae0c">It makes you see the world in a different light.</p>



<p id="306d">It humbles you.</p>



<p id="3e69">And if you let it, it can open you.</p>



<p id="5dc5">Sometimes I think the best journeys aren’t measured in miles, but in mindset.</p>



<p id="52ec">You don’t have to get on a plane.</p>



<p id="9e71">You just have to&nbsp;<em>notice</em>&nbsp;something new.</p>

<p id="f775">And in that moment, amid machines and masks and schedules, we were both simply&nbsp;<em>human</em>.</p>

<p id="ec71">When I turned 50, I didn’t care how many birthdays I had left.</p>



<p id="2fdb">I cared whether I could still carry my groceries. Climb stairs. Finish a sentence without losing the thread</p>



<p id="794c">I cared how many&nbsp;<em>good</em>&nbsp;years I had left.</p>



<p id="11fd">Not lifespan. Healthspan.</p>



<p id="86f2">I wanted to know how long I could stay sharp, strong, and independent. Not just alive, but&nbsp;<em>thriving</em>. So I started asking a new question of my patients, my research, and myself:</p>

<p id="6762">What’s the one blood test that tells me how fast I’m aging?</p>
</blockquote>



<p id="ce01">Most people think it’s cholesterol. Or maybe blood sugar. But those are lagging indicators. They tell you what’s broken, not what’s brewing.</p>



<p id="3cc9">The test I care about most now?</p>



<p id="1877"><strong>Hs-CRP.</strong></p>



<p id="b6cf">High-sensitivity C-reactive protein.</p>



<p id="031f">Hs-CRP levels predict more than inflammation — they can forecast your future health. The higher the number, the shorter the path to chronic disease.</p>



<p id="9f0b">It’s not flashy. It doesn’t trend on social media.</p>



<p id="8a1e">However, it may be the most important number you’re not tracking.</p>



<h1 class="wp-block-heading" id="6e0e"><strong>Why Inflammation Matters More Than You Think</strong></h1>



<p id="a5b6">Hs-CRP is a marker of inflammation.</p>



<p id="daca">And inflammation, more than almost anything else, is what turns time into damage.</p>



<p id="b1fb"><mark>Chronic inflammation accelerates heart disease. It promotes cancer. It fuels Alzheimer’s, frailty, and age-related decline.</mark></p>



<p id="52ac">You don’t need a PhD to understand this: aging well means inflaming&nbsp;<em>less</em>.</p>



<p id="598e">Hs-CRP doesn’t measure one disease. It measures your body’s silent alarm system.</p>



<p id="b0e0">And when it’s elevated, things are already smoldering.</p>



<p id="6274">In healthy adults, hs-CRP should be below 1.0 mg/L.</p>



<p id="f326">Between 1 and 3 is a moderate risk. Anything above 3 is a warning sign.</p>



<p id="d742">Here’s the problem: Most people don’t know their number. And most doctors don’t order it unless you’ve already had a heart attack.</p>



<p id="97c1">But if you care about your healthspan, you should.</p>

<p id="072e">Medicine tends to focus on what’s urgent, not what’s important.</p>



<p id="50f8">Hs-CRP doesn’t diagnose a specific disease. It doesn’t tell you what organ is failing. It just whispers: something’s wrong.</p>



<p id="b95a">And in modern medicine, whispers get ignored.</p>



<p id="289c">In the U.S., insurance may not routinely cover it unless you’re already at high cardiovascular risk.</p>



<p id="dbfa">Clinical guidelines don’t push it for prevention.</p>



<p id="5589">And most physicians are too busy putting out fires to go looking for smoke.</p>



<p id="f7f1">But that’s exactly what this test reveals: slow, quiet inflammation that may not make headlines, but shortens your healthspan all the same.</p>



<p id="c218"><strong>Can You Order hs-CRP Without a Doctor?</strong>&nbsp;Yes — in most U.S. states, you can. Services like Ulta Lab Tests, Request A Test, or Walk-In Lab allow you to order an hs-CRP online for $30–$70. You choose a local draw site, such as Quest or Labcorp. No doctor visit required.&nbsp;<em>Exceptions: New York, New Jersey, and Rhode Island restrict consumer lab orders.</em></p>



<h1 class="wp-block-heading" id="c03f"><strong>What My Patients Taught Me</strong></h1>



<p id="e1a8">Some of my fittest-looking patients had elevated hs-CRP.</p>



<p id="58c4">Not because they were overweight.</p>



<p id="b553">But because they were inflamed.</p>



<p id="edbb">Sleep deprivation. Chronic stress. Processed foods. Environmental toxins. Dental infections. Even loneliness.</p>



<p id="a9c9">Inflammation has many faces.</p>



<p id="4cb9">And that’s what makes hs-CRP so powerful: it doesn’t just reflect one system. It integrates them all.</p>

<p id="cff5">The liver makes C-reactive protein in response to inflammation. But it’s not just a random flare-up detector — it’s a proxy for systemic stress.</p>



<p id="0df5">Numerous studies have shown that oxidative stress is linked to accelerated telomere shortening and dysfunction. Oxidative stress caused by inflammation, cell factors, or environmental exposures contributes to degenerative diseases and cancer.</p>

<h1 class="wp-block-heading" id="0a15"><strong>What Happened When I Lowered Mine</strong></h1>



<p id="217a">The first time I measured my hs-CRP, it was 2.9.</p>



<p id="4687">I was sleeping 5 hours a night, skipping meals between consults, and not drinking enough water.</p>



<p id="8561">Nothing looked wrong on paper. But I felt off—mentally slower, physically stiff, emotionally flat.</p>



<p id="4e62">Six months later, I made three changes:</p>



<ul>
<li>I walked every morning before checking my phone.</li>



<li>I swapped protein bars for real food.</li>



<li>I prioritized sleep as if it were a prescription.</li>
</ul>



<p id="3b03">My hs-CRP dropped to 0.7.</p>



<p id="a987">My brain felt clearer.</p>



<p id="d4d0">My joints were less inflamed.</p>



<p id="46a9">Even my mood improved.</p>

<p id="db40">No drug magically cures inflammation. But lifestyle can.</p>



<p id="b577">What works?</p>

<li><strong>Fiber</strong> (especially soluble fiber from legumes, chia seeds, oats, and vegetables</li>



<li><strong>Omega-3s</strong> (like those from fish or algae oil)</li>



<li><strong>Sleep</strong> (7–8 hours, not negotiable)</li>



<li><strong>Walking</strong> (daily, ideally in nature)</li>



<li><strong>Resistance training</strong> (2–3x/week)</li>



<li><strong>Social connection</strong> (yes, seriously)</li>



<li><strong>Oral hygiene</strong> (brush, floss, and keep your mouth free of inflammation — it’s your body’s open door to systemic disease)</li>



<li><strong>Screen-free wind-down time</strong> (to calm your nervous system)</li>

<p id="e938">I check my hs-CRP every 6 months now. Not because I’m afraid of dying.</p>



<p id="8c71">But because I want to live well.</p>



<h1 class="wp-block-heading" id="27ce">The Textbook Answer</h1>



<p id="8c3e">For most healthy individuals,&nbsp;<strong>routine C-reactive protein (CRP) testing is not necessary</strong>.</p>



<p id="1e3a">However, there are some exceptions where it can be useful, especially&nbsp;<strong>high-sensitivity CRP (hs-CRP)</strong>, which can assess&nbsp;<em>chronic, low-grade inflammation</em>, a known risk factor for heart disease and other conditions.</p>



<h1 class="wp-block-heading" id="d4a9">Here’s a breakdown of when it might be useful:</h1>



<h2 class="wp-block-heading" id="6e58">You might consider hs-CRP testing if you:</h2>



<ul>
<li>Have a <strong>family history of heart disease</strong> or early cardiovascular events.</li>



<li>Have <strong>metabolic syndrome</strong> or multiple cardiovascular risk factors (e.g., high blood pressure, diabetes, smoking, obesity).</li>



<li>Have <strong>autoimmune symptoms</strong> (e.g., fatigue, joint pain, skin issues) and your doctor is evaluating for inflammatory conditions.</li>



<li>You are already diagnosed with <strong>heart disease</strong> or <strong>chronic inflammatory conditions</strong> (like rheumatoid arthritis or lupus), and your provider uses CRP to monitor disease activity.</li>
</ul>



<h2 class="wp-block-heading" id="272f">You probably don’t need CRP testing if:</h2>



<ul>
<li>You’re <strong>young, healthy, and asymptomatic</strong> with no notable risk factors.</li>



<li>You’re not going to change your treatment plan based on the result (i.e., no action would be taken).</li>
</ul>



<h1 class="wp-block-heading" id="8b7b"><strong>Final Thoughts</strong></h1>



<p id="c0fb">You can’t choose your genes. But you can choose your daily fire level.</p>



<p id="4f5b"><em>Inflammation is optional.</em></p>



<p id="e963">Hs-CRP is the one number I now track more than cholesterol, blood pressure, or glucose.</p>



<p id="6616">Because it reveals how much damage I’m silently absorbing.</p>



<p id="e055">And how much resilience I still have left.</p>

<p id="1daa">This week, a patient shared a poignant realization that emerged after years of emotional distance from his wife.</p>



<p id="9d16">They had grown apart, but recently discovered a shared need: the simple, profound act of touch.</p>



<p id="43b0">He reflected on how a gentle hug or a reassuring hand on the shoulder seemed to bridge the emotional gap between them.</p>



<p id="ae50">“I think we’re wired for this,” he mused, referencing hormones like oxytocin that respond to physical affection.</p>



<p id="5f1c">His insight aligns with scientific findings.</p>

<p id="0e14">In fact, research indicates that even brief moments of affectionate touch can lead to measurable increases in oxytocin, a hormone that contributes to an improved mood and reduced anxiety.</p>



<p id="017f">This finding underscores the biological underpinnings of our&nbsp;<a href="https://elifesciences.org/articles/81241?utm_source=chatgpt.com" rel="noreferrer noopener" target="_blank">need for physical connection</a>.</p>



<p id="4f0d">My patient’s experience serves as a reminder that sometimes, healing in relationships doesn’t require grand gestures — just a touch of understanding, quite literally.</p>

<p id="c74a">In oncology, gratitude comes in many forms — thank-you notes, quiet nods, even tears.</p>



<p id="d2ba">But sometimes, it arrives as a steaming pan of pasta.</p>



<p id="4778">One of our patients, an older Italian gentleman with a twinkle in his eye and impeccable taste, has taken to feeding the staff.</p>



<p id="7e28">Not metaphorically — literally.</p>



<p id="b7c8">Lasagna, tiramisu, and even delicate cannoli are dusted with sugar like freshly fallen snow.</p>



<p id="87a8">Today, he arrived bearing a new dish. “Pasta Puttanesca!” he announced proudly. “You know —&nbsp;<strong>the prostitute’s pasta.</strong>”</p>



<p id="edad">A pause.</p>



<p id="d99e">Then laughter. Nurses chuckled. My medical assistant nearly dropped her stethoscope.</p>



<p id="dd8d">He winked. “They say it was made quickly, between clients.”</p>

<p id="d7d6">This event was something else entirely in a world often defined by scans and side effects.</p>

<p id="c045">Most days, I aim radiation at tumors.</p>



<p id="454c">But the most profound insights sometimes come in the quiet moments between treatments.</p>



<p id="a964">I see a lot of overworked patients.</p>



<p id="ffe0">And now, neuroscience confirms what many of us have long suspected:</p>



<p id="6448"><strong>Overwork doesn’t just wear us down — it reshapes who we are, starting with the brain.</strong></p>



<p id="250c">A&nbsp;<a href="https://oem.bmj.com/content/82/3/105" rel="noreferrer noopener" target="_blank">recent study</a>&nbsp;has illuminated something startling and validating:</p>



<h1 class="wp-block-heading" id="718f">The Study That Turned Heads</h1>



<p id="fd7d">A groundbreaking study published in&nbsp;<a href="https://oem.bmj.com/content/early/2025/05/08/oemed-2025-110057" rel="noreferrer noopener" target="_blank"><em>Occupational and Environmental Medicine</em></a>&nbsp;analyzed MRI scans of 110 healthcare workers.</p>



<p id="6488">It found something startling:</p>



<p id="8e96">Compared to those with standard hours, those clocking more than 52 hours a week showed&nbsp;<strong>structural changes in 17 brain regions.</strong></p>



<p id="d523">These regions govern critical functions like decision-making, memory, attention, emotional regulation, and planning.</p>



<p id="ffc7"><a href="https://www.cnn.com/2025/05/13/health/long-working-hours-study-scli-intl-wellness" rel="noreferrer noopener" target="_blank">Jonny Gifford</a>&nbsp;of the Institute for Employment Studies called the study “powerful new evidence,” explaining that these changes may explain why long hours erode judgment and resilience.</p>



<p id="3659">Yes, the sample was small — and limited to South Korean healthcare workers — but MRI-based neuroimaging gives it scientific weight.</p>



<p id="3d44">Here’s where the changes show up — and why they matter.</p>



<h1 class="wp-block-heading" id="edb2">What Does This Mean?</h1>

<p id="0ff9">The affected brain areas include:</p>



<ul>
<li><strong>Middle Frontal Gyrus</strong>: Working memory and attention (think: forgetting why you opened a tab).</li>



<li><strong>Insula</strong>: Emotional regulation and empathy (that short fuse? It’s not just you).</li>



<li><strong>Superior Temporal Gyrus</strong>: Processing sound and language (why everything feels “too loud” after a long day).</li>
</ul>



<p id="9ef6">These changes suggest chronic overwork can reduce emotional stability, increase anxiety, and strain our closest relationships.</p>

<p id="d3b4">His 2021&nbsp;<a href="https://www.who.int/news/item/17-05-2021-long-working-hours-increasing-deaths-from-heart-disease-and-stroke-who-ilo" rel="noreferrer noopener" target="_blank">WHO-ILO study</a>&nbsp;estimated that long working hours contribute to&nbsp;<strong>745,000 deaths annually</strong>, making them the world’s leading occupational risk factor.</p>



<p id="b5bd">Some brain areas may enlarge under stress, but growth isn’t always resilience.</p>



<p id="92b2"><strong>Sometimes it’s your brain shouting for help.</strong></p>



<h1 class="wp-block-heading" id="b213">The Brain’s Attempt to Adapt</h1>



<p id="7d8d">These changes aren’t just damage — they might be&nbsp;<strong>neuroadaptation</strong>, the brain’s desperate attempt to cope.</p>



<p id="c41d">But that adaptation comes at a cost.</p>



<ul>
<li>Emotional dysregulation.</li>



<li>Cognitive fatigue.</li>



<li>Burnout.</li>
</ul>



<h1 class="wp-block-heading" id="dccb"><em>Can the Brain Bounce Back?</em></h1>

<p id="ea36">But only if you give it time, space, and support.</p>
