<p id="adfe">Last week, in the final days of the campaign, and making his closing argument for a second term,&nbsp;<a href="https://www.nytimes.com/2020/10/30/us/elections/after-trump-accuses-doctors-of-profiteering-medical-professionals-push-back.html">President Donald Trump said this at one of his rallies</a>:</p>



<blockquote class="wp-block-quote is-style-default td_pull_quote td_pull_center is-layout-flow wp-block-quote-is-layout-flow"><p>You know our doctors get more money if somebody dies from Covid. With us, when in doubt, choose Covid.</p></blockquote>



<p id="6c6a">How dare you, Sir?</p>



<p id="b7fe">Over these past eight months, we on the front lines have witnessed truly horrible things. We have seen death and destruction on a scale like nothing else we have seen in our careers. We have placed our own lives — and those of our families — on the line taking care of the myriad patients who have been afflicted by this disease (including, by the way, the doctors at Walter Reed Army Medical Center).</p>



<p id="4e33">How dare you accuse us of faking Covid deaths for “more money”? That is not what we do. I have signed dozens of death certificates after patients died of Covid-19, and I have also signed death certificates of patients who did not have Covid. Never — not even once — did I think about putting down Covid-19 as a cause of death when the patient was free of Covid. Never even once.</p>



<p id="988a">We don’t have time to worry about inflating deaths from Covid to “make the President look bad.” We don’t have time to think about “faking a death certificate” for political reasons. We are too focused on doing all that we can to help our patients survive Covid-19. And if they can’t survive, then we are focused on making sure they have a <a href="https://elemental.medium.com/a-good-death-28673f68d60c">good death</a>.</p>



<p id="303e">After everything we have gone through in this pandemic, this baseless accusation was particularly infuriating. The&nbsp;<a href="https://coronavirus.jhu.edu/map.html">staggering number of deaths&nbsp;</a>from Covid-19 in this country — over 230,000 — is due to a staggering failure by our leadership to control this pandemic. It has absolutely nothing to do with doctors “faking Covid deaths.”</p>

<p id="65d9">President Trump <a href="https://www.nytimes.com/2020/10/07/us/politics/trump-coronavirus-blessing.html">stated</a> in a video that contracting Covid-19 was a “blessing from God.</p>

<p id="e74f">He called it a blessing from God because he was able to get multiple treatments for COVID-19. He then went on to tout those treatments as a “cure” for the disease. The reality, however, is quite different. The treatments he received can hardly be characterized as “cures,” and here is the data behind each one.</p>



<h1 class="wp-block-heading" id="2321">Monoclonal Antibodies</h1>



<p id="b3b7">The most high profile treatment President Trump received — under a “compassionate use” application — was monoclonal antibodies. These are proteins that are highly specific against SARS CoV-2, and they have the ability to latch on to the virus to prevent it from entering the cells.</p>



<p id="82d7">The <a href="https://investor.regeneron.com/news-releases/news-release-details/regenerons-regn-cov2-antibody-cocktail-reduced-viral-levels-and">data behind using them</a> is from a study of <em>ambulatory</em> patients, which means patients in a doctor’s office or clinic setting, not the hospital. Right off the bat, this tells me that those patients were not very sick, to begin with. If they were, they would have been hospitalized. The drug has not been studied in that patient population, as far as I know.</p>



<p id="058a">In that ambulatory setting, the greatest benefit was for those patients who did not have their own antibodies against the disease, so-called seronegative patients. Those who had antibodies already did not benefit as much. Furthermore, there is absolutely no evidence that this treatment will work in patients with severe disease requiring hospitalization or ICU care (such as patients whom I see).</p>



<p id="0721">What’s more, it seems that the highest dose of the antibodies — the same dose the President received — was not as effective as the lower dose. More of the patients receiving the highest dose had to seek medical attention, and it took them longer to achieve symptom alleviation. These findings, however, were not statistically significant.</p>



<p id="8ad7">So, really, this treatment is hardly a “cure” as the President claims. It is definitely promising, but we need to wait for more data to come out.</p>



<h1 class="wp-block-heading" id="5ca9">Remdesivir</h1>



<p id="52e1">President Trump also received remdesivir, an anti-viral drug that was originally developed for Ebola. Remdesivir may also be helpful in treating Covid-19. The final results were recently <a href="https://www.nejm.org/doi/full/10.1056/NEJMoa2007764">published in the New England Journal of Medicine</a>.</p>



<p id="dc6f">All that remdesivir does is shorten the time to recovery from 15 days to 10 days compared to placebo. That’s it. I am not saying that this is not significant, but it is not a “cure.”</p>



<p id="1db9">Now, there was a trend toward lower mortality in the patients who received the drug, but technically, it was not statistically significant. Thus, you can’t make this claim about Remdesivir, and the study authors did not make this claim in their study conclusions.</p>



<h1 class="wp-block-heading" id="b000">Dexamethasone</h1>



<p id="d6b6">By far, the&nbsp;<a href="https://jamanetwork.com/journals/jama/fullarticle/2770277">best data</a>&nbsp;is for dexamethasone. In this randomized clinical trial, the number of days alive and free from mechanical ventilation during the first 28 days was significantly higher among patients treated with dexamethasone plus standard care when compared with standard care alone (6.6 days vs 4.0 days).</p>



<p id="1858">When they&nbsp;<a href="https://jamanetwork.com/journals/jama/fullarticle/2770279">pooled all the data</a>&nbsp;for steroids in a meta-analysis, the 28-day all-cause mortality was lower among patients who received corticosteroids compared with those who received usual care or placebo (summary odds ratio, 0.66). This odds ratio means that the group who got steroids had a 34% decreased risk of death.</p>



<p id="ef1a">Now, the President received other treatments such aspirin, famotidine, zinc, and vitamin D. The data for these treatments is even more sparse. That’s what makes it so frustrating with this disease: it such a horrific condition with few truly effective treatment options.</p>



<p id="5341">The treatments for seasonal influenza are also not blockbusters, but seasonal influenza is not nearly as deadly as COVID-19. So, at the end of the day, the studies behind the treatments that the president received were not blockbusters, and they showed some benefit. But, I would hardly characterize them as “cures.”</p>

<p></p>



<p>Finding a magic cure is a surefire way to get reelected in the middle of a global pandemic. The entire world is waiting for a reliable vaccine or an effective medication to treat Covid-19.</p>



<p>China and Russia have already approved the limited use of COVID-19 vaccines without clinical trial data. What is to stop President Donald Trump from setting up a perfect “made for television” moment by releasing a Covid-19 vaccine just before the election?</p>



<p>Donald Trump is likely to release a Covid-19 vaccine under <a href="https://www.fda.gov/regulatory-information/search-fda-guidance-documents/emergency-use-authorization-medical-products-and-related-authorities#liability" rel="noreferrer noopener" target="_blank">the emergency use authorization</a> (EUA) before completing phase 3 data indicating the vaccine is safe and effective. He is expected to release the vaccine in October.</p>



<p>An October surprise would look good on TV but not permit enough time to discover if the vaccine works or causes harm before we cast our votes.</p>



<h4 class="wp-block-heading">What is the evidence for EUA vaccine&nbsp;release?</h4>



<p>The Trump administration has tested the waters. Recent EUAs have been approved for <a rel="noreferrer noopener" href="https://www.fda.gov/media/137566/download" target="_blank">Remdesir</a>, <a rel="noreferrer noopener" href="https://www.fda.gov/news-events/press-announcements/fda-issues-emergency-use-authorization-convalescent-plasma-potential-promising-covid-19-treatment" target="_blank">Convalescent Plasma, </a>and Rapid Antigen testing. The FDA revoked a previously EUA for Hydroxychoroquin after studies showed the benefits did not outweigh the risk.</p>



<p>An August 27th letter from CDC director Robert Redfield urges state governors to expedite “applications for these distribution facilities and, if necessary, asks that you consider waiving requirements that would prevent these facilities from becoming fully operational by November 1, 2020.”</p>



<p>The US government has partnered with McKesson Corp. to distribute coronavirus vaccines to state and local health departments. We have also invested in eight different COVID-19 vaccines.</p>



<p>Stephen Hahn, the FDA director, published an article in JAMA stating a COVID-19 vaccine will be reviewed “under either the traditional Biologics License Application (BLA) review process or under the Emergency Use Authorization (EUA) program.”</p>



<p>The writing is on the wall for the Covid-19 EUA vaccine release.</p>



<h4 class="wp-block-heading">How EUA&nbsp;works?</h4>



<p>Countries like the United States create regulatory pathways for emergency use of vaccines and therapeutics outside of a clinical trial in the event of a public health, military, or national security emergency.</p>



<p>The EUA grants liability protection to pharmaceutical companies. The EUA also ends once the emergency or threat has resolved.</p>



<p>FDA director Stephen Hahn set the goals for a Covid-19 vaccine in a Washington Post Op-Ed stating, “any authorized or approved Covid-19 vaccine would need to show that it prevents the disease or decreases its severity in at least 50 percent of people who are vaccinated.”</p>



<h4 class="wp-block-heading">What is the harm with releasing a Covid-19 vaccine under the&nbsp;EUA?</h4>



<p>Andy Slavitt, the former Medicare and Medicaid director for President Barack Obama summarized the issue stating, “Done right, vaccines end pandemics. Done wrong, pandemics end vaccines.”</p>



<p>Vaccines require trust. We rely on regulatory agencies to oversee the clinical trials to ensure a potential vaccine is safe and effective. If we can not trust our regulatory agencies to rely on science and protect the public from political pressure, we have a potential disaster recipe.</p>



<p>Various governmental agencies carefully govern clinical trials. Dating back to the infamous <a href="https://www.cdc.gov/tuskegee/timeline.htm" rel="noreferrer noopener" target="_blank">Tuskagee study</a> in which black men were injected with syphilis without consent, strict guidelines were put into place for ethical clinical research.</p>



<p>Drug trials go through <a href="https://www.fda.gov/patients/drug-development-process/step-3-clinical-research" rel="noreferrer noopener" target="_blank">three phases:</a></p>



<ol class="wp-block-list"><li><strong>Phase 1</strong> trials focus on safety and dosage.</li><li><strong>Phase 2 t</strong>rials evaluate efficacy and side effects.</li><li><strong>Phase 3</strong> trials are large trials seeking to prove a drug or vaccine works better than what is already available and to confirm <a href="https://www.fda.gov/patients/drug-development-process/step-3-clinical-research#collapse3" rel="noreferrer noopener" target="_blank">efficacy and monitor for adverse reactions</a>.</li></ol>



<p>Releasing a vaccine without proving it does what it is supposed to do and does not cause harm is crucial to establishing trust. Without validation, we risk losing the public trusts in a Covid-19 vaccine as well as vaccines in general.</p>



<p>The World Health Organization (WHO) <a href="https://www.thelancet.com/journals/lancet/article/PIIS0140-6736%2820%2931821-3/fulltext" rel="noreferrer noopener" target="_blank"><strong>Solidarity Vaccines Trials Expert Group</strong></a> highlighted the dangers in an editorial published in The Lancet. An unproven or harmful vaccine could worsen the pandemic.</p>

<h4 class="wp-block-heading">Vaccine makers pledge not to see FDA approval without&nbsp;data</h4>



<p>In a bizarre twist, vaccine manufacturers have jointly pledged not to send a vaccine to the FDA approval without safety and efficacy data. Pfizer Inc., Moderna Inc., Johnson &amp; Johnson, GlaxoSmithKline Plc, Sanofi, and potentially others are among the companies working together to avoid losing the public trust.</p>



<p>This joint pledge implies the vaccine manufacturers believe the FDA under the Trump administration can not be considered reliable to oversee the approval process.</p>



<p>Vaccine makers are taking a public stance against getting their own product to market.</p>



<h4 class="wp-block-heading">Rushing medication to market can have grave consequences</h4>



<p>The entire world waits on pins and needles for a magic bullet to stop COVID-19 dead in its tracks. We cheer on researchers working around the clock to find a solution, a vaccine, or a cure.</p>



<p>Despite our enthusiasm, we must take a cautious approach to medical research. We can not lose the public’s trust.</p>



<p>The FDA and CDC have made crucial mistakes cowering to political pressure. It is not too late to let the career scientists lead the way.</p>
